吻合器直肠黏膜环切术结合外痔切除缝合术治疗重度混合痔的临床疗效

摘要

目的 探讨吻合器直肠黏膜环切术(PPH)结合外痔切除缝合术治疗重度混合痔的临床疗效.方法 选择PPH结合外痔切除缝合治疗的70例Ⅲ～Ⅳ度混合痔患者,并与同期外剥内扎手术治疗的70例Ⅲ～Ⅳ度的混合痔患者作比较研究,观察两组手术时间、术后并发症、住院时间、治愈时间及住院费用.结果 两组在手术时间、术后并发症、术后住院时间、治愈时间及住院费用等方面比较差异有统计学意义(P＜0.05),治疗组疗效明显优于对照组,但费用较高.结论 与传统手术相比,PPH结合外痔切除缝合术具有手术时间短、疼痛轻、创伤小、恢复快等优点,是治疗重度混合痔患者理想方法.但由于住院费用明显高于传统手术,所以在临床上的推广应用受到限制.%Objective To investigate the clinical effect of PPH combined with external hemorrhoid resection and suture on hemorrhoids.Methods Thirty-five cases of Ⅲ-Ⅳ degree mixed hemorrhoids treated by PPH combined with external hemorrhoid resection and suture were compared with other 35 cases treated by external dissection and internal legations at the same period.Postoperative complications,operational time,hospitalization time,cure time and hospitalization expenditure of the two groups were observed.Results There were significant diffcrcnces in incidence of operation time,postoperative complications,cure time and hospitalization time between the two groups (P ＜ 0.05).The therapeutic effect of the treatment group was better than that of the control group,but the hospitalization expenditure of the treatment group was higher than that of the control group.Conclusion PPH combined with external hemorrhoidectomy is an optimal surgical option in the treatment of severe hemorrhoids and is proved to have the advantage of shorter operative time,lighter pain,less surgical damage,faster recovery and less complication,in comparison with the conventional surgical method.But the hospitalization expenditure is higher than that in the control group obviously,so the application of this technique in clinics is not popularized.
