目的 探讨高频超声动态观察妊娠期乳腺良性肿瘤的发展变化及其评价合理干预肿瘤最佳手术时机的价值.方法 对186例妊娠期乳腺良性肿瘤患者定期行高频超声检查,动态观察肿块的变化.对接受妊娠期非手术治疗患者每隔2个月复查超声,对接受手术治疗者于术后1周、2周、1个月、2个月复查超声.结果 186例中,105例肿块较小,增长速度缓慢、无变化或消失；78例肿块较大(超过20 mm),多在孕22～30周增长较迅速,30周后增长速度逐渐缓慢；3例肿块生长迅速者术后病理确定为恶性.超声动态观察发现于妊娠18～24周手术治疗并发症较少,患者预后良好.结论 高频超声可动态观察妊娠期乳腺肿块的发展变化,有助于选择最佳手术时机.%Objective To explore the value of high-frequency ultrasound for observation of development of benign breast tumors during pregnancy and evaluation on the best operation time. Methods Totally 186 patients with non-pathological pregnancy breast benign tumor underwent high-frequency ultrasonic examination, and the changes of lesion were observed dynamically. For patients underwent non-operative treatment during pregnancy, ultrasonic following-up was performed every 2 months, while for patients underwent surgical operation, ultrasonic following-up was performed 1 week, 2 weeks, 1 month, 2 months after operation. Results In 186 patients, there were 105 cases with small lesions growing slowly or did not grow, 78 cases with bigger masses (>20 mm) which growing relatively rapidly during 22-30 weeks and then gradually slow down. The masses grew rapidly and were pathologically confirmed malignancy after surgical resection in 3 cases. Dynamic ultrasound found that surgical operation performed during pregnancy in 18-24 weeks had fewer complications, and the prognosis was good. Conclusion High-frequency ultrasound can observe the changes of breast tumors during pregnancy, and is helpful to selecting optimal operation time.
