肝细胞癌(HCC)是世界范围内的第5大常见肿瘤,且男性肿瘤病死率居第3位.病毒性肝炎、肥胖和代谢综合征是HCC发病的明确危险因素.本文关注最新数据和推荐指南,回顾性分析HCC的诊断和治疗方法.HCC早期诊断依赖于动态加强的典型放射影像,肝移植、手术切除和射频消融是HCC的治愈性疗法,而索拉非尼和经动脉化疗性栓塞多推荐用于晚期HCC,由肝脏病学、外科学、放射学、肿瘤学和病理学专家组成的多学科综合治疗是未来HCC治疗的发展方向.%Hepatocellular carcinoma (HCC) is the fifth common cancer and the third important cause of cancer related death for men in the world.Viral hepatitis,obesity and metabolic syndrome are wellestablished risk factors contributing to the long-term rising incidence of HCC.In this paper,we focus on the most recent evidences and recommendations from guidelines and review the diagnostic and therapeutic management of HCC.The early diagnosis of HCC rely on typical radiological hallmarks in dynamic contrastenhanced imaging.Liver transplantation,surgical resection and radiofrequency ablation are considered to be cornerstones of curative therapy,while sorafenib and transarterial chemoembolization are recommended for patients with advanced HCC.A multidisciplinary team consisting of hepatologists,surgeons,radiologists,oncologists and pathologists is fundamental for the correct management of HCC.
