中西医结合治疗肺源性心脏病急性发作期的临床观察

     

摘要

目的:探讨中西医结合疗法对 肺源性心脏病(肺心病)的疗效。方法:86例肺心病急性发作期患者随机分成2组,43例对照组患者采用吸O2、抗生素、解痉平喘、强心、利尿、纠正电解质紊乱和酸碱失衡等治疗。治疗组43例患 者 给以口服慢阻通气丸、抗咳喘丸,同时静滴清开灵注射液、丹参注射液、维脑络通,必要时 吸O2,注意纠正酸碱失衡和电解质紊乱。2组疗程均14日。结果:治疗组总有效率为88.3 7%,显著高于对照组58.14%(χ2=10.50,P<0.01)。治疗组治疗后血气分析 动脉 氧分压(PaO2)、pH值升高,二氧化碳分压(PaCO2)下降;肺功能中肺活量(VC),第1秒用力呼气容积(FEV1)及用力 肺活量(FVC)增加,较对照组均有显著改善(P均<0.05)。结论:中西医结合疗法具有改善血气指标和肺功能,提高临床疗效的作用。%Objective:To investigate the curative effects of integrated traditional Chinese medicine and western medicine on cor pulmonale( CP).Methods:Eightysix cases with CP who were in episodic sta ge were randomly divided into two groups (43 cases for each group).The control group was treated with symptomatic therap ies,including inhaling oxygen,antiinfective,inotropic,diuretic,relie ving bronchial spasm and wheeze,correcting electrolyte disorder and acid base imbalance and so on,otherwise the treated group treated with Manzu Tongq i pill(MZTQP,慢阻通气丸) and Ka ngkechuan pill(抗咳喘丸) taken orally meanwhile Qingkailing inject ion(清开灵注射液),salvia milt iorrhiza injecton,and venoruton infused intravenously once a day .If necessary inhaling oxygen and correcting electrolyte disorder and acidbase imbalance had been used.Fourteen days as a therapeuti c course to both groups.Results:The total effective rate(88.37%) in treated group was si gnificantly higher than that(58.14%) in control group(χ2=10.50,P<0. 01). Furthermore in treated group after therapy the arterial oxygen pressure(PaO2) and pH were obviously elevated,partial pressure of carbon dioxide(PaCO2) depressed.Regarding the pulmonary functions the vital capacity(VC),forced expiratory volume in 1 second,forced vital capacity(FVC) were significantly imp roved and in comparison with control group the differe nces were significant(all P<0.05).Conclusions:Integrated traditional Chinese medicine and western medi cine therapy is able to improve pulmonary functions and para meters of blood gas analysis so that may highten the clinically curative effects.
