胰岛素泵治疗重症糖尿病患者120例临床分析

摘要

目的：观察胰岛素泵与多次皮下注射胰岛素治疗重症糖尿病的效果与安全性。方法将120例重症糖尿病患者随机分为观察组与对照组，每组各60例。观察组患者采用胰岛素泵持续皮下输注重组人胰岛素注射液治疗，对照组患者采用重组人胰岛素注射及甘精胰岛素强化治疗，比较治疗前后两组患者血糖水平变化、维持达标血糖胰岛素用量、血糖达标时间和低血糖发生情况。结果治疗前两组患者空腹血糖（FPG）、餐后2小时血糖（2hPG）及睡前血糖水平比较差异均无显著性（P＞0.05），治疗后上述指标均显著下降，两组间比较差异均具有显著性（P＜0.05）。观察组患者血糖达标时间短于对照组，维持达标血糖胰岛素用量低于对照组（P＜0.05）。两组患者低血糖发生率比较差异具有显著性（χ2＝3.96，P＝0.009）。结论胰岛素泵与多次皮下注射胰岛素对重症糖尿病患者的血糖控制均具有较好的疗效和安全性，但胰岛素泵更利于患者血糖控制和减少并发症。%ObjectiveTo observe the clinical efifcacy and safety of continuous subcutaneous insulin infusion (CSII) and multiple subcutaneous insulin injection (MSII) in the treatment of server diabetes.Method120 patients with server diabetes mellitus were selected and randomly divided into observation group and control group, 60 cases in each group, observation group patients were treated with CSII, and control group patients were treated with MSII. Change of blood-glucose, standard time of blood-glucose, insulin dosage of maintaining standard blood-glucose and incidence rate of hypoglycemia were contrasted between the two groups.Result The levels of fasting blood-glucose (FPG), 2 hours postprandial blood glucose (2hPG) and bedtime blood glucose had no signiifcant differences between the two groups before treatment (P>0.05), and these indexes were decreased in the two groups after treatment, the difference was significant between the two groups (P<0.05). The standard time of blood-glucose was shorter and insulin dosage of maintaining standard blood-glucose was lower in observation group than control group (P<0.05), and the incidence rate of hypoglycemia was signiifcant different between the two groups (χ2=3.96,P=0.009).ConclusionBoth CSII and MSII have well curative effect and security in the treatment of server diabetes mellitus, but CSII is preponderant in controlling blood-glucose and decreasing complications.
