彩色多普勒超声检测高血压病肾动脉血流动力学的临床研究

         

摘要

目的:探讨彩色多普勒超声检测高血压肾动脉血流动力学的诊断意义。方法收集就诊于江苏省昆山市第六人民医院超声科的临床诊断为高血压的患者96例作为观察组,同时收集该院进行健康体检的患者98例作为对照组,对所有研究对象均采用彩色多普勒超声进行检测,对两组患者的肾脏长径、实质厚度、肾窦厚度、实质厚度/肾窦厚度的比值以及肾窦部的段动脉中段和皮髓交界处的叶间动脉段血流各项参数进行比较。结果观察组患者的肾脏长径、实质厚度、肾窦厚度、实质厚度/肾窦厚度比值均高于对照组(P<0.05),观察组患者肾窦部的段动脉中段的收缩期最大血流速度(Vmax)、舒张期最低血流速度(Vmin)、阻力指数(RI)值低于对照组(P<0.05),观察组对象的叶间动脉段的Vmax、Vmin、RI值低于对照组,差异具有统计学意义(P<0.05)。结论彩色多普勒超声可以检测高血压病肾动脉血流动力学各项参数,可反映继发性肾灌注的改变,为临床诊断高血压病的早期肾功能损害提供了重要依据。%Objective To investigate the diagnostic significance of color Doppler ultrasonography for detecting renal artery hemodynamics of hypertension.Methods The patients with hypertension were chosen as observation group (n=96) and the objects with physical examinations were chosen as control group (n=98) from the Department of Ultrasonography of Sixth People’s Hospital of Kunshan City in Jiangsu Province. All objects were given the detections of color Doppler ultrasonography, and some indexes including renal length, renal parenchymal thickness, thickness of renal sinus and ratio of parenchymal thickness to sinus thickness, and hemodynamic parameters of middle segment of sinus renal artery (SRA) and segment of interlobular renal artery (IRA) at cortex-medulla junction were compared in 2 groups.Results The indexes of renal length, renal parenchymal thickness, thickness of renal sinus and ratio of parenchymal thickness to sinus thickness were all higher in observation group than those in control group (P<0.05). The maximum systolic flow velocity (Vmax), minimum diastolic flow velocity (Vmin) and resistance index (RI) were all lower in observation group than those in control group (P<0.05). The values of Vmax, Vmin and RI of IRA were all lower in observation group than those in control group (P<0.05).Conclusion Color Doppler ultrasonography can be used for detecting hemodynamic parameters of hypertensive renal arteries, which can reflect the changes in secondary renal perfusion and provide important evidence in diagnosis of early renal damage in patients with hypertension.
