血清内脏脂源性丝氨酸蛋白酶抑制剂水平与2型糖尿病大血管病变的相关性分析

     

摘要

Objective To explore the association of serum vaspin level with the T2DM macrovascular complications. Methods Sixty-five newly diagnosed T2DM patients were divided into simple T2DM group (SDM group, n=33) , and T2DM macrovascular complication group (MV group, n = 32). Thirty healthy people with normal glucose tolerance were selected as the controls (NC group). Serum vaspin was measured by ELISA method. FPG, 2 hPG, Fins, HbA1c hsC-RP, and lipid profiles were measured as well. BMI and waist-to-hip ratio (WHR) were recorded and insulin sensitivity was assessed by HOMA model. Results The serum vaspin levels of three groups were in order of MV group (0. 46±0. 24) < NC group (0. 49 ± 0. 21) < SDM group (0. 67±0. 39). In the SDM group, the vaspin level was positively correlated with sex, BMI, Fins, TG, HOMA-IR, hsC-RP, and WHR (r=0.561, 0.653, 0.632, 0.578, 0.516, 0.698, 0.625, P<0. 05 or F<0. 01), and negatively correlated with HDL-C (r= —0. 647, P<0. 01). In the MV group, the vaspin level was negatively correlated with SBP, HOMA-IR, and hsC-RP (r= —0. 592, -0. 526, —0. 628, P<0. 05 or P<0. 01) , and positively with sex (r=0532, P<0. 05). Logistic regression analysis showed that TG, BMI, and the fasting serum vaspin levels were the main risk factors for macrovascular complications in T2DM. Conclusion Vaspin plays a very important role in type 2 diabetes mellitus with macrovascular complications.%目的 探讨T2DM患者血清丝氨酸蛋白酶抑制剂(vaspin)水平与大血管病变的相关性.方法 选择新诊断T2DM患者(65例),根据有无血管病变分为单纯2型糖尿病组(SDM组,33例),2型糖尿病合并大血管病变组(MV组,32例),选择健康体检者为对照组(NC组,30名);采用ELISA方法测定空腹血清vaspin水平,检测血脂、HbA1c、FPG、hsC-RP等指标,采用稳态模式评估HOMA-IR. 结果 3组血清vaspin水平依次为:MV组(0.46±0.24)<NC组(0.49±0.21)<SDM组(0.67±0.39); SDM组血清vaspin水平与性别、BMI、WHR、HOMA-IR、FIns、HDL-C、TG、hsC-RP有显著相关性(r值分别为0.561、0.653、0.625、0.516、0.632、-0.647、0.578、0.698,P<0.05或P<0.01).MV组vaspin与SBP、HOMA-IR、hsC-RP、性别有显著相关性(r=-0.592、-0.526、-0.628、0532,P<0.05或P<0.01).Logistic回归分析显示:vaspin、TG、BMI是T2DM大血管病变的主要危险因素. 结论 vaspin在T2DM大血管病变的发生中起重要作用.
