2型糖尿病患者葡萄糖负荷后氨基酸水平变化及其与胰岛素抵抗的关系

         

摘要

Objective To evaluate the changes of serum amino acid level during 75 g OGTT and their relationship with insulin resistance in type 2 diabetes. Methods A total of 3 6 hospitalized patients with type 2 diabetes were enrolled in this study. Blood samples were collected before and 2 hours after glucose load during OGTT. Serum amino acids were measured by targeted LC-MS/MS. Results Val [(278. 32±57. 30)vs (245. 10±51. 65)μmol/L],Leu[(229. 45±42. 29)vs (184. 10±47. 19)μmol/L],Ile [(107. 03±36. 24)vs (85. 72±37. 39)μmol/L],Tyr[(66. 50±12. 17)vs (55. 50±13. 04)μmol/L]and Phe[(81. 00 ± 20. 49 )vs (69. 57 ± 17. 35 )μmol/L]significantly decreased after 75 g glucose load. Correlation analysis showed that the changes of Val,Ile and Phe after glucose load were positively associated with fasting insulin and HOMA-IR. Conclusion Serum amino acid levels significantly decrease after 75 g glucose load. Additionally,the higher level of fasting insulin and HOMA-IR are,the lower level of Val,Ile and Phe will be after glucose load.%目的:研究T2DM患者在行75 g OGTT过程中血清氨基酸水平的变化及其与IR的关系。方法选取36例于我院内分泌科住院的T2DM患者。收集行75 g OGTT过程中0 h及2 h血清样本。用液质联用技术(LC-MS/MS)检测血清氨基酸水平。结果与OGTT 0 h比较,OGTT 2 h缬氨酸[Val,(278.32±57.30)vs (245.10±51.65)μmol/L]、亮氨酸[Leu,(229.45±42.29)vs (184.10±47.19)μmol/L]、异亮氨酸[Ile,(107.03±36.24)vs (85.72±37.39)μmol/L]、酪氨酸[Tyr,(66.50±12.17)vs (55.50±13.04)μmol/L]、苯丙氨酸[Phe,(81.00±20.49)vs (69.57±17.35)μmol/L]水平下降(P<0.001)。Pearson相关分析显示,Val差、Ile差、Phe差与FIns、HOMA-IR呈正相关。结论葡萄糖负荷后,血清氨基酸水平下降。FIns越高,HOMA-IR越大,葡萄糖负荷后Val、Ile、Phe下降越明显。
