脉搏血氧饱和度监测在老年慢性心脏疾病患者中的应用

摘要

目的：探讨脉搏血氧饱和度监测在老年慢性心脏疾病患者护理干预中的作用。方法：将复旦大学附属中山医院2014年6月—12月收治的100例老年慢性心脏疾病患者随机分为观察组和对照组，各50例。对照组采用常规方法测量脉搏和呼吸，观察组在常规护理基础上采用脉搏经皮血氧饱和度仪监测脉搏血氧饱和度，并根据监测结果进行分级护理干预。比较2组急性心衰发生率、护理满意度，并对脉搏血氧饱和度与动脉血氧饱和度的相关性进行分析。结果：观察组急性心衰发生率显著低于对照组，护理满意度显著高于对照组，差异均有统计学意义（P＜0．05）；Pearson分析显示，脉搏血氧饱和度与动脉血氧饱和度测值的相关性好（r＝0．922，P＜0．01）。结论：用脉搏血氧饱和度仪测量脉搏血氧饱和度可使护理更加规范、高效，有利于急性心衰的早期发现。%Objective:To investigate the role of pulse oxygen saturation monitoring in nursing intervention for elderly patients with chronic heart disease.Methods:A total of 100 elderly patients with chronic heart disease treated in Zhongshan Hospital,Fudan University from June to December 2014 were randomly divided into observation group and control group(50 patients in each group).Pulse and respiration were measured by conventional methods in the control group while on the basis of conventional nursing,pulse oxygen saturation was measured by pulse oximeter in the observation group,and different grades of nursing intervention were carried out according to the monitoring results.The incidence rate of acute heart failure and nursing satisfaction were compared between the two groups,and the correlation between pulse oxygen saturation and arterial oxygen saturation was analyzed.Results:The incidence rate of acute heart failure was significantly lower in the observation group than that in the control group,nursing satisfaction was significantly higher than that in the control group,and the differences were statistically significant (P<0.05).Pearson analysis showed that pulse oxygen saturation was highly correlated with arterial oxygen saturation (r= 0.922,P<0.01).Conclusions:Using pulse oximeter to measure pulse blood oxygen saturation can make nursing more standardized and efficient,which is beneficial for the early detection of acute heart failure.
