1540nm非剥脱点阵铒玻璃激光治疗黄褐斑的疗效观察

     

摘要

Objective To evaluate the efficacy and safety of nonablative 1 540nm fractional photothermolysis for the treatment of Melasma. Methods 16 patients (Fitzpatrick skin types IV~V) with Melasma were enrolled.the lesions were treated with 1,540nm fractional photothermolysis.spot size 5mm,pluse width 15ms,fluence 4~10mJ/mb,5~6pass.Ten treatment sessions were performed for each patient 4 weeks apart.Comparative photographs were taken using VISIA specific complexion analysis to identify and quantify spots. Results 16 cases of Melasma obtained different degree of improvement were treated by 10 times.VISIA absolute scores declined significantly from the baseline to the follow up visits of each treatment.3(18.7%) patients obtained 65% improvements ,7 (43.7%) patients obtained 60% improvements. All improve rate was 58.7% and none side effect besides two case hyperpigmentation.and it has obviously resurfacing effect. Conclusions StarLuxTM 1 540 nm fractional laser is effective and safe in the treatment of melasma.the side effects was rare, without downtime almost, which deserves the clinical expansion.%目的:观察1540nm非剥脱点阵铒玻璃激光治疗黄褐斑的临床疗效和安全性.方法:选取16例面部黄褐斑患者,采用1 540hm点阵激光治疗,光斑15mm,脉宽15ms,能量4~ 10mJ/mb(微光束microbeam),扫描3~5次,共治疗10次,每4周1次,治疗后严格防晒.采用VISIA图像分析系统和前后照片比较改善效果.结果:16例经过10次治疗,对比治疗前后照片和VISIA图像分析系统统计分析,黄褐斑均有不同程度改善,其中3例(18.7%)达到65%以上的改善率,7例(43.7%)达到60%以上的改善率,总改善率58.7%.除2例发生色素沉着外没有色素减退等严重副作用.同时还有收缩毛孔、改善皮肤纹理的作用.结论:1 540hm点阵激光治疗黄褐斑疗效确切,安全性高,无明显副作用,为治疗黄褐斑提供了一个新手段.
