掌背动脉岛状皮瓣与腹部皮瓣修复手指背侧皮肤软组织缺损的临床疗效对比分析

摘要

Objective To compare the clinical effects of dorsal metacarpal artery island lfap and abdominal lfap in the repair of soft tissue defect of ifnger dorsum.Methods 200 patients with ifnger soft tissue defects in our hospital from April 2014 to January 2016 were selected. The patients were divided into observation group and control group (100 cases in each group) according to different repair methods. A total of 101 ifngers in the observation group were treated with dorsal metacarpal artery island lfap and 101 ifngers in the control group was treated with abdominal lfap. The excellent expression rate, operation time, hospital stay, hospitalization costs and adverse reactions occurred during the 6 months after operation were compared between the two groups.Results The excellent and good rate of observation group was signiifcantly higher than that in control group (88.12%vs 66.34%) (χ2=14.103,P<0.05). The operation time had no signiifcant difference between the two groups [(53.64±6.57)min vs (57.37±8.64)min] (P>0.05) and the hospitalization time and hospitalization costs in observation group were signiifcantly less than those in the control group[(7.14±2.34)d,(0.74±0.14)wan vs (12.02±4.37)d,(1.24±0.34)wan] (P<0.05).There were no obvious adverse reactions in two groups.ConclusionThe curative effect of dorsal metacarpal artery island lfap in the repair of soft tissue defect of ifnger dorsum is better than that of abdominal lfap. The treatment of dorsal metacarpal artery island lfap has curative effect,postoperative lfap survival rate, short hospitalization time, less surgical expenses and no obvious adverse reactions, which is worth popularizing in clinical use.%目的:对比分析掌背动脉岛状皮瓣与腹部皮瓣对修复手指背侧皮肤软组织缺损的治疗疗效.方法:选取本院2014年4月至2016年1月收治的200例手指背侧皮肤软组织缺损患者.按照不同修复方式将患者分成观察组与对照组(每组100例).观察组共101指采用掌背动脉岛状皮瓣修复,对照组共101指采用腹部皮瓣修复.比较两组患者术后6个月优良率,以及手术时间、住院天数、住院费用、不良反应发生情况.结果:术后观察组患者优良率(88.12%)明显高于对照组(66.34%)(χ2=14.103,P<0.05);两组患者手术时间比较无差异[(53.64±6.57)min vs(57.37±8.64)min](P>0.05),观察组患者住院时间以及住院费用明显少于对照组[(7.14±2.34)d、(0.74±0.14)万vs(12.02±4.37)d、(1.24±0.34)万](P<0.05);两组患者均未出现明显不良反应情况.结论:掌背动脉岛状皮瓣疗效优于腹部皮瓣治疗疗效,具有术后皮瓣存活率高,患者住院时间短,手术经费少的特点,并且无明显不良反应,值得在临床上推广使用.
