Objective To develop a new home hospice service mode to satisfy the current demands of the advanced cancer patients and evaluate its efficacy. Methods 70 advanced cancer patients at home in Desheng community were selected from January,2010 to January,2013 as the research subjects. We provided them with home hospice care service,and then ana-lyzed their benefits,cost saving and satisfaction. Results We had set up a home hospice service mode,a remote video conferen-cing platform and developed two electronic management systems. Among the 70 patients with terminal cancer,18(25. 7% )were lung cancers,60(85. 7% )were patients with severe or moderate pain and the mean survival time(MST) from diagnosis to death was 30 months. The daily average hospitalization expense in Community Health Center in the end - period was 1 / 13 of that in 3 A grade hospitals(243: 3 126). 69 patients(98. 6% )were satisfied with the new service model. Conclusion This home hospice care service model in Desheng community has better effects in terms of benefits and cost saving and can meet the basic de-mands of home terminal patients so it accords with the present condition of community hospice care service in our country and has popularizing values.%目的：探索癌症晚期患者居家临终关怀服务模式，并对其进行效果评价。方法选取2010年1月-2013年1月德胜社区居家癌症晚期患者70例为研究对象，对其实施临终关怀服务，并对其获益情况、成本节约情况、满意度等进行分析。结果建立社区居家临终关怀服务模式，建立远程视频会诊平台、开发两大电子管理软件。在管理的癌症晚期患者中，肺癌患者18例（占25.7%），中重度疼痛患者60例（占85.7%），患者从癌症确诊到死亡平均生存期为30个月。临终于社区的患者日均住院费用与临终于三甲医院的相比为1：13（243：3126）。患者总体满意度达98.6%（69例）。结论德胜社区居家临终关怀模式在患者获益、成本节约方面效果较好，满足了居家临终患者的根本需求，是符合我国社区居家临终关怀的服务模式，具有很好的推广价值。
