南京西路街道社区2型糖尿病患者自我血糖监测的现状及干预研究

摘要

目的：了解南京西路街道社区2型糖尿病患者自我血糖监测(SMBG)现状，并探讨综合性干预对自我血糖监测的影响和效果。方法：339例调查对象被随机分为对照组和干预组，对照组不进行任何干预措施，干预组进行综合性干预措施，比较两组自我血糖监测的认知、行为及血糖控制效果。结果：339例患者中，便携式血糖仪拥有率38.3%，自我血糖监测率37.2%，自我血糖监测频率4.5次/月。未进行自我血糖监测的原因中前三位是“因经济原因不愿使用血糖仪(81.2%)”“自己认为没有必要监测(71.4%)”和“不会使用血糖仪监测血糖(47.4%)”。干预后，干预组对自我血糖监测的认知率、支持率、检测率及血糖达标率均高于对照组(P＜0.05)。结论：南京西路街道社区2型糖尿病患者自我血糖监测的水平仍较低，需强化对糖尿病及自我血糖监测的认识，帮助患者掌握血糖监测技术，通过综合性干预提高自我血糖监测的依从性，有效改善社区糖尿病的管理和随访工作。%Objective:To explore the status of self monitoring blood glucose(SMBG) in patients with type 2 diabetes mellitus in nanjing west road street community,and to explore the effect of comprehensive intervention on self monitoring blood glucose. Methods:339 cases were randomly divided into the control group and the intervention group.The control group did not have any intervention.The intervention group used comprehensive intervention measures.We compared the cognitive,behavioral of self monitoring blood glucose and blood glucose control effect of the two groups.Results:In 339 patients,the rate of having portable blood glucose meter was 38.3%;the rate of self monitoring blood glucose was 37.2%;the frequency of self monitoring blood glucose was 4.5 times/month.In the reasons of without self monitoring blood glucose,the former three respectively were "due to economic reasons,they were unwilling to use blood glucose meter(81.2%)","they didn't think it is necessary(71.4%)" and "they will not use the blood glucose meter to monitor blood glucose(47.4%)".After intervention,the cognitive rate,support rate,detection rate of self monitoring blood glucose and blood sugar compliance rate of the intervention group were higher than those in the control group(P<0.05).Conclusion:The level of self monitoring blood glucose in patients with type 2 diabetes mellitus in nanjing west road street community was still lower.We need to strengthen the understanding of diabetes and self monitoring of blood glucose, help patients to master blood glucose monitoring technology,improve the compliance of self monitoring of blood glucose by comprehensive intervention,so as to effectively improve the community diabetes management and follow-up work.
