2 h后血气分析结果对新生儿窒息程度评估的探讨

         

摘要

目的:探讨新生儿窒息程度评估中采用2 h后血气分析的意义。方法129例窒息新生儿分为轻度窒息组(76例)与重度窒息组(53例),均取新生儿桡动脉血行血气分析,并以100例健康新生儿作为对照组,对比其血气分析结果与脏器情况。结果轻度、重度窒息组动脉血二氧化碳分压(PaCO2)及酸碱度(pH值)明显高于对照组,动脉血氧分压(PaO2)及剩余碱(BE值)明显低于对照组,差异有统计学意义(P<0.05);轻度窒息组PaCO2明显低于重度窒息组,而PaO2、pH值及BE值明显高于重度窒息组,差异有统计学意义(P<0.05);轻度窒息组脏器损伤率46.1%明显低于重度窒息组的92.5%,差异有统计学意义(P<0.05)。结论2 h后血气分析结果可为新生儿窒息程度与脏器损伤程度的判断以及临床诊疗提供可参考的依据,意义重大。%Objective To investigate the significance of applying blood gas analysis after 2 h in evaluation of neonatal asphyxia.Methods A total of 129 infants with neonatal asphyxia were divided into mild asphyxia group (76 cases) and severe asphyxia group (53 cases), and their radial artery blood was taken for blood gas analysis. There were 100 healthy infants in control group. Their blood gas analysis results and organ condition were compared.Results The mild and severe asphyxia groups had higher arterial partial pressure of carbon dioxide (PaCO2) and potential of hydrogen (pH) value and lower arterial partial pressure of oxygen (PaO2) and base excess (BE) values than the control group, and their differences had statistical significance (P<0.05). The mild asphyxia group had obviously lower PaCO2 than the severe asphyxia group, while its PaO2, pH and BE values were all higher than the severe asphyxia group. The difference had statistical significance (P<0.05). The organ injury rate was 46.1% in the mild asphyxia group, which was much lower than 92.5% in the severe asphyxia group, and their difference had statistical significance (P<0.05).Conclusion Blood gas analysis after 2 h can provide reference for neonatal asphyxia, organ injury and clinical treatment, along with important significance.
