X线造影在早期食管癌诊断分析中的作用

摘要

目的 探讨X线在早期食管癌诊断分析中的作用.方法 选取2013年6月~2014年6月期间在我院接受治疗的早期食道癌患者50例,所使用诊断仪器:上海XG-300D型医用X线诊断机,Toshiba影像增强电视系统对患者进行钡餐造影检查,回顾患者的基本临床资料及X线片诊断结果,对X线在早期食管癌诊断分析中的作用进行合理评价.结果 根据X线片诊断特征对患者进行划分,其中表现为黏膜皱襞改变患者共15例,其主要表现特征为局限性黏膜走行不稳,多变,黏膜皱褶增粗,迂回,中断,边缘毛糙,呈网状或颗粒状;表现出充盈缺损症状患者20例,其主要表现特征为肿物表现光滑、息肉样肿物突向管腔内,充盈缺损大小在0.5cm×0.5cm~1.0cm×1.0cm之间,基底部较为狭窄;表现出龛影患者13例,其主要表现特征为癌组织向腔内生长,边缘较不规则,底部可见溃疡,且溃疡大小凹凸不平、大小不等,约在0.2cm×0.2cm~0.5cm×0.5cm之间,形态不规则;表现出管壁改变患者共12例,其主要特征为管壁毛糙、不规则轮廓、且扩张程度较差;表现出管腔狭窄患者5例,其主要特征为病变呈局限性,且向心性,部分患者有时可见轻度痉挛.结论 对早期食管癌患者实施X线片诊断具有较好的临床效果,且该种诊断法操作简单,诊断准确有效,但在诊断过程中,对于检查医师及责任医师的要求较高,相关医师需对食管黏膜特征等进行充分掌握,以预防漏诊或误诊事件的发生.%Objective To study the effect of X-ray contrast on diagnostic analysis of early esophageal cancer.Methods50 patients with early esophageal cancer,who were treated in our hospital from June 2013 to June 2014,were selected and made barium meal examination with Toshiba radiography enhancing TV system,with the diagnosis instrument of type Shanghai XG-300D X-ray diagnosis machine.To retrospectively analyze the clinical data and the X-ray diagnosis results,to make X-ray rational evaluation on the analysis of early esophageal cancer.ResultsAccording to the X-ray diagnostic characteristics of patients,it was showed that,there were 15 patients with alterant mucosal fold,with the primary expressive characteristics of unstable line feed and mutable on boundedness mu cosa,thickening,circuitous,interrupt,rough edge,reticular or granular on mucosal reductus;There were 20 patients with filling defect,with the primary expressive characteristics of glabrous goitre,polypoid goitre process to lumen,the size of filling defect of 0.5cm×0.5cm-1.0cm×1.0cm,relatively stegnotic basal part; there were 12 patients with alterant tube wall,with the primary expressive characteristics of crude tube wall,irregular contour,poorer dilation degree;There were 5 patients with luminal stenosis,with the primary expressive characteristics of boundedness and centrality pathology,mild spasm in some patients.Conclusion X-ray contrast on the diagnosis of early esophageal cancer has better clinical effect,simple operation,accurate and effective diagnosis,has higher requirements on examination doctor and charge doctor,who should fully grasp the characteristics of esophageal mucosa to prevent the occurrence of missed diagnosis and misdiagnosis.
