Objective To explore the influence of Bezafibrate on endothelial function,insulin resistance and blood pressure in hypertensive patients combine with hypertriglyceridemia.Methods 150 patients with hypertension and hypertriglyceridemia were divided into observation group and control group according to the treatment methods in Traditional Chinese Medicine Hospital of Botou City from December 2011 to January 2013,75 cases in each group,the control group was given conventional treatment,the observation group was given Bezafibrate treatment on the basis of the control group,the changes of total cholesterol (TC),three acyl glycerin (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),uric acid,vascular endothelial active factors,uric acid and insulin resistance were compared between the two groups.Results The ET,NO and TXB2/PGF1α in the observation group after 8 weeks treatment,the differences were statistically significant compared with before treatment (P ＜ 0.05); the NO obvious rise [(28.5±10.2) μmmol/L],ET [(82.4±18.1) pg/mL],TXB2/PGF1α (2.1±0.9) decreased obviously in observation group after 8 weeks treatment; while the control group had no significant change.The systolic and diastolic blood pressure were lower after treatment between the two groups,8 weeks after treatment compared with before treatment,the differences were statistically significant (P ＜ 0.05),there was no statistically significant difference between the two groups (P ＞ 0.05).The TC,TG,LDL-C,blood glucose and uric acid were reduced after treatment in the observation group,while the ISI and HDL-C were rised,the differences were statistically significant compared with before treatment (P ＜ 0.05).The TC,TG,LDL-C declined slightly,while the blood sugar,uric acid and ISI did not change significantly,the differences were not statistically significant (P ＞ 0.05).The TC,TG,HDL-C,LDL-C,blood glucose,ISI and uric acid were compared between the two groups,the differences were statistically significant (t =3.97,9.25,3.94,3.89,3.91,4.03,10.14,all P ＜ 0.05).Conclusion Bezafibrate adjustable blood fat in hypertensive patients combined with hypertriglyceridemia,improve endothelial function and insulin resistance in patients,diastolic blood pressure have obvious effect on the patients.%目的 探讨苯扎贝特联合降压对高血压合并高甘油三酯血症患者内皮功能、胰岛素抵抗和血压的影响.方法 将泊头市中医院2011年12月～2013年1月收治的150例高血压合并高甘油三酯血症患者,按照治疗方法分为观察组与对照组,每组各75例,对照组患者给予常规治疗,观察组在对照组的基础上给予苯扎贝特治疗,观察并比较两组患者的总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、尿酸、血管内皮活性因子、尿酸和胰岛素抵抗的变化.结果 观察组治疗后8周内皮素(ET)、一氧化氮(NO)和血栓烷B2(TXB2)/前列腺素F1α(PGF1α)与治疗前比较,差异有统计学意义(P＜0.05).观察组治疗8周后NO明显上升[(28.5±10.2)μmol/L],ET明显下降[(82.4±18.1)pg/mL],TXB2/PGF1α明显下降(2.1±0.9);而对照组无明显变化.治疗后两组患者的收缩压和舒张压均降低,治疗8周后与治疗前比较,差异有统计学意义(P＜0.05),两组间比较,差异无统计学意义(P＞0.05).观察组患者治疗后TC、TG、LDL-C、血糖和尿酸均降低,而胰岛素敏感指数(ISI)和HDL-C上升,与治疗前比较,差异有统计学意义(P＜0.05);对照组患者的TC、TG、LDL-C轻微下降,而血糖、尿酸和ISI变化不明显,差异无统计学意义(P＞ 0.05).TC、TG、HDL-C、LDL-C、血糖、ISI及尿酸比较,差异有统计学意义(t=3.97、9.25、3.94、3.89、3.91、4.03、10.14,均P＜0.05).结论 苯扎贝特可调节高血压合并高甘油三酯血症患者的血脂,改善患者的血管内皮功能和胰岛素抵抗,对患者的舒张压有明显的作用.
