微切口超声乳化加非球面人工晶体植入治疗白内障的临床评价

         

摘要

Objective To analyze and explore the clinical effect of micro-incision falsification atmospheric binocular lens implantation treating cataract. Methods From January 2010 to January 2014, in Fenghua Hospital of Traditional Chinese Medicine, 120 patients with cataract were selected and divided into observation group and control group, ac-cording to the treatment plan, with 60 cases in each group. The observation group was given micro-incision falsification atmospheric IOLs (Zeiss atmospheric IOL Acri Smart 36A) implantation therapy, the control group was given micro-in-cision falsification spherical binocular lens (IOL spherical Zeiss, CT Spheris 209M) for treatment. Except IOL model, the remaining treatment options and care methods were the same. The vision levels, 3 mm pupil straight higher-order aberrations and complications were observed. Results The follow-up were 5-11 months, the average was (7.57±2.12) months. One days after operation, in the observation group, vision more than 1.0 were 9 cases, vision 0.6-1.0 were 26 cases, vision 0.3-0.5 were 25 cases; in the control group, vision more than 1.0 were 8 cases, vision 0.6-1.0 were 25 cases, vision 0.3-0.5 were 27 cases. Three months after operation, in the observation group, vision more than 1.0 were 15 cases, 0.6-1.0 were 34 cases, vision 0.3-0.5 were 11 cases;in the control group, vision more than 1.0 were 13 cases, vision 0.6-1.0 were 33 cases, vision 0.3-0.5 were 14 cases. After 1 days and 3 months after operation, the vision levels of patients in two groups were compared, the differences were not statistically significant (P> 0.05). In the ob-servation group, corneal high order aberration was (0.035±0.012), lens higher-order aberrations was (0.050±0.017), oc-ular higher order aberrations was (0.036±0.012); in the control group, corneal high order aberration was (0.051±0.017), lens for higher-order aberrations was (0.077±0.019), ocular higher order aberrations was (0.074±0.024), the high order aberration in the observation group were significantly lower than those in the control group, the differences were statis-tically significant (P<0.05). In the observation group, postoperative corneal edema was 1 cases, 1 cases was transient high binocular pressure, 2 cases were incision infection, the complication rate was 6.67%; in the control group, 2 cases were postoperative corneal edema, 3 cases were transient high binocular pressure, 2 cases were incision infection, the complication rate was 11.67%, the complication rates of two groups were compared, the differences were not statistically significant (P>0.05), and all patients recovered well after treatment. Conclusion The micro-incision falsification and binocular lens implantation in the treatment of non-spherical micro-incision cataract falsification compares with spher-ical binocular lens implantation for cataract, it can get better restore vision, reduce higher-order aberrations, and it is worth to be promotion and use.%目的:探讨微切口超声乳化加非球面人工晶体植入治疗白内障的临床效果。方法选取2010年1月~2014年1月在奉化市中医医院接受治疗的白内障患者120例。根据治疗方案将其分为观察组与对照组,每组各60例。观察组采用微切口超声乳化联合非球面人工晶体(蔡司非球面人工晶状体Acri Smart 36A)植入进行治疗,对照组采用微切口超声乳化联合球面人工晶状体(蔡司球面人工晶状体CT Spheris 209M)进行治疗。除人工晶状体型号不同,其余治疗方案及护理方法均相同。观察两组视力水平、3 mm瞳孔直高阶像差以及并发症发生情况。结果随访5~11个月,平均(7.57±2.12)个月。术后1 d,观察组视力1.0以上9例,视力0.6~1.026例,视力0.3~0.525例;对照组视力1.0以上8例,视力0.6~1.025例,视力0.3~0.527例。术后3个月,观察组视力1.0以上15例,视力0.6~1.034例,视力0.3~0.511例;对照组视力1.0以上13例,视力0.6~1.033例,视力0.3~0.514例。术后1 d和术后3个月,两组患者视力水平比较,差异无统计学意义(P>0.05)。观察组角膜高阶像差为(0.035±0.012),晶状体高阶像差为(0.050±0.017),全眼高阶像差为(0.036±0.012);对照组角膜高阶像差为(0.051±0.017),晶状体高阶像差为(0.077±0.019),全眼高阶像差为(0.074±0.024),观察组各项高阶像差均明显低于对照组,差异有统计学意义(P<0.05)。观察组术后角膜水肿1例,一过性眼压高1例,切口感染2例,并发症发生率为6.67%;对照组术后角膜水肿2例,一过性眼压高3例,切口感染2例,并发症发生率为11.67%,两组患者并发症发生率比较,差异无统计学意义(P>0.05),且经过处理后均恢复良好。结论微切口超声乳化加非球面人工晶体植入治疗白内障较微切口超声乳化联合球面人工晶状体植入治疗白内障能够较好地恢复视力,有效降低高阶像差,可以在临床上进一步推广和使用。
