标本溶血对生化检验结果的影响及对策

摘要

Objective To research the effect and strategy of sample hemolysis on the biochemical test results. Methods 68 cases of normal people receiving the physical examination in our hospital from April 2015 to July 2016 were selected as the research objects and the indexes including ALB, CK, HDL, TG, CK-MB, AST, LDH, TP, K+, HBDH and CHO before and after the routine biochemical test hemolysis were tested by the automatic analyzer, and the change of various indexes before and after the hemolysis was compared. Results The measurement values of CK,CK-MB,AST,LDH,TP,K+,HBDH after hemol-ysis were higher than those before hemolysis, and the differences had statistical significance (P<0.05), and there was no ob-vious change in the ALB,HDL,TG,CHO, and the difference had no statistical significance(P>0.05), which indicated that CK, CK-MB,AST,LDH,TP,K+,HBDH were related to the hemolysis. Conclusion The sample hemolysis has an obvious effect on multiple biochemical test items such as CK,CK-MB,AST,LDH,TP,K+,HBDH, and the medical staff should standardize the operation when collecting the blood, and they can use the serum hemoglobin for proofread.%目的：研究标本溶血对生化检验结果的影响并分析对策。方法选择2015年4月-2016年7月在该院接受体检的68例正常人作为此次研究对象。该次研究使用全自动分析仪进行常规生化检测溶血前后的指标包括清蛋白(ALB)、肌酸激酶（CK）、高密度脂蛋白（HDL）、三酰甘油（TG）、肌酸激酶同工酶（CK-MB）、天门冬氨酸氨基转移酶（AST）、乳酸脱氢酶（LDH）、总蛋白质（TP）、血钾（K+）、α-羟基丁酸脱氢酶（HBDH）以及总胆固醇（CHO）。对比溶血前后各项指标发生的变化。结果溶血前后的各项生化指标对比，其中CK、CK-MB、AST、LDH、TP、K+、HBDH的测定值，溶血后高于溶血前，比较差异有统计学意义（P<0.05）。ALB、HDL、TG、CHO等未见明显变化，比较差异无统计学意义（P>0.05）。说明CK、CK-MB、AST、LDH、TP、K+、HBDH等的指标和溶血相关。结论标本溶血对多数生化检验项目如CK、CK-MB、AST、LDH、TP、K+、HBDH有明显的影响，要求医护工作者在采血时规范操作，严格把关。可利用血清血红蛋白（Hb）浓度校对。
