剖宫产围术期预防性应用抗生素预防术后感染效果

         

摘要

目的:探讨剖宫产围术期预防性应用抗生素防止术后感染的效果。方法选择该院2013年11月-2014年3月产科分娩的剖宫产产妇386例随机分为观察组(n=193)和对照组(n=193),观察组于术前30 min快速静脉滴注头孢唑林钠,术后12 h 内再同等剂量给药1次;对照组术后回病房静脉滴注头孢唑林钠,连用3~5 d。观察比较两组产妇术后体温、切口感染率、子宫复旧不良率、术后住院时间。结果观察组产妇术后体温正常时间为(34.25±7.67)h,对照组为(35.31±8.36)h,两组比较差异无统计学意义(P>0.05);观察组产妇切口感染率、子宫复旧不良率、术后住院时间分别为1.55%、3.11%、(5.3±1.2)d,对照组分别为6.21%、8.81%、(7.6±1.5)d,两组比较差异有统计学意义(P<0.05)。结论剖宫产围术期术前应用抗生素用药剂量少、用药时间短、无不良反应、术后恢复快,预防术后感染具有显著效果。%Objective To investigate the effect of preventative application of antibiotics in the perioperative period of cesarean section on preventing postoperative infection. Methods 386 puerperants with cesarean section admitted to our hospital from November 2013 to March 2014 were selected and randomly divided into the observation group and control group, with 193 cases in each group. The observation group was given rapid intravenous drip cefazolin sodium 30 min before operation, and then given the same dose within 12 h after the operation. The control group was given cefazolin sodium intravenously for 3-5 d after the op-eration. The postoperative body temperature, wound infection rate, poor uterine involution rate and postoperative hospital stay were observed and compared between the two groups. Results The time of returning to normal body temperature after the operation of the observation group was (34.25±7.67)h, and that of the control group was (35.31±8.36) h, the difference between the two groups was not statistically significant(P>0.05); The wound infection rate, poor uterine involution rate and postoperative hospital stay were significantly better than those of the control group [1.55% Vs. 6.21%;3.11% Vs. 8.81%; (5.3±1.2)d Vs.(7.6±1.5)d; the differences were statistically significant (P<0.05). Conclusion Preventative application of antibiotics in the perioperative period of cesarean section has many advantages such as less antibiotics dosage, short administration time, no adverse reactions and rapid postopera-tive recovery, which has a significant effect on preventing postoperative infection.
