彩色多普勒超声联合MRI对小乳腺癌的诊断价值分析

         

摘要

目的:探讨彩色多普勒超声联合MRI对小乳腺癌的诊断价值。方法方便选择自2010年1月―2016年1月在该院确诊为小乳腺癌(病灶最大直径≤2 cm﹚的89例患者(均为单侧发病﹚为研究对象,所有患者均接受乳腺彩色多普勒超声及MRI检查,比较两种方法分别诊断及联合诊断小乳腺癌的临床价值。结果89个乳腺癌肿块中直径≤1.0 cm共36例,1.0 cm<直径≤2.0 cm共53例。对于直径≤1.0 cm的乳腺癌肿块,超声与MRI联合检查(94.44%﹚的检出率明显高于单独应用超声(75.00%﹚或MRI(77.78%﹚检查,差异均有统计学意义(与联合诊断相比,P<0.05﹚;而对于1.0 cm<肿块直径≤2.0 cm的乳腺癌患者来说,超声与MRI联合检查(96.23%﹚的检出率虽高于单独应用超声(88.68%﹚或MRI(90.57%﹚检查,但是差异无统计学意义(与联合诊断相比,P>0.05﹚。肿块在彩色多普勒超声图像上表现为圆形或椭圆形,边界清晰或模糊,肿块内血流呈现短线状,直径较小者血流分级多为Ⅰ级,直径较大者血流分级多为Ⅱ~Ⅲ级。血流阻力指数平均(0.73±0.08﹚。在MRI图像上部分肿块边缘不清,可见毛刺征;T1加权成像呈现低信号或等信号,部分肿块T2加权像上呈现混杂高信号,随着肿块直径的增大,其强化曲线多表现为Ⅱ、Ⅲ型。结论彩色多普勒超声诊断小乳腺癌准确率高,且简便、经济,可作为筛查小乳腺癌的首选,联合MRI诊断小乳腺癌的价值更优。%Objective To evaluate the value of color Doppler ultrasound combined with MRI in the diagnosis of small breast cancer. Methods Convenient selection from January 2010 to January 2016 diagnosed in our hospital for small breast carcinoma (maximum lesion diameter is less than or equal to 2 cm) of 89 cases of patients (unilateral) as the object of study, all patients underwent breast color Doppler ultrasound and magnetic resonance imaging (MRI), to compare the two methods respectively diagnosis and combined the clinical value of small breast cancer diagnosis. Results 89 breast cancer tumors less than 1.0 cm in diameter with a total of 36 cases, 1.0cm 0.05). Tumor in color Doppler ultrasound image manifestations were round or oval in shape, the boundary clear or fuzzy, tumor blood flow showed short-term shaped, smaller diameter flow grade for grade I, larger diameter flow grade for grade II - III. Mean blood flow resistance index (0.73 ±0.08). In MRI images on the part of the mass with ill defined margins and visible spiculation; T1 weighted imaging showed low signal or signal, the part of the mass on T2 weighted images showed mixed high signal, with the increase of the diameter of the tumor, the enhancement curve more performance for the type II and III. Conclusion Color Doppler ultrasound in the diagnosis of small breast cancer accuracy is high, and simple, economical, can be used as the first choice for screening small breast cancer, combined with the value of MRI diagnosis of small breast cancer is more excellent.
