经腹股沟腹膜前间隙UHS三维网片治疗腹股沟疝疗效分析

摘要

Objective To compare the clinical effects of Rutkow, UHS procedures on inguinal hernia repair. Methods Thirty patients received mesh plug tension free hemioplasty ( Rutkow procedure) while the other twenty-nine patients suffering from the transinguinal preperitoneal groin hernia repair used a partially adsorbable three-dimensional preperitoneal mesh named UHS procedure. The operation time, postoperative pain, time of hospital stay, time to return to work, cost, postoperative complication and recurrent rate were compared between the two groups. Results There was no significant difference in postoperative pain, time of hospital stay, time to return to work, postoperative complication and recurrent rate (P >0.05). However,Rutkow group had a significantly lower cost than UHS (P < 0.01). Rutkow group had a higher incidence of foreign body sensation than UHS ( P =0.002). Conclusion The two types of tension-free her-nioplasty are all effective in treatment of primary monolateral inguinal hemia. UHS hemioplasty is the choice for repair of primary inguinal hernia which is more comfortable while the patients have a good finacial condition and pursue a higher level of life quality.%目的 研究开放式经腹股沟入路腹膜前间隙UHS三维网片无张力疝修补手术临床效果.方法 30例采用疝环充填式无张力疝修补术(Rutkow术式),29例采用腹膜前间隙UHS修补术.比较两种手术方法 在手术时间、手术后疼痛、住院时间、住院费用、恢复工作时间、术后并发症和复发率的差别.结果 两组患者在手术后疼痛、恢复工作时间、术后并发症和复发率差异均无统计学意义(P>0.05).在住院费用上Rutkow组低于UHS组(P<0.01).在术后异物感方面Rutkow组明显高于UHS组(P=0.002).结论 两种无张力疝修补术治疗单侧原发性腹股沟疝疗效肯定.其中,UHS组术后异物感轻,如病人费用容许,术后生活质量要求高者,建议应用UHS术式.
