目的：探讨2型糖尿病合并冠心病患者血清载脂蛋白 B（apoB）／载脂蛋白 AI（apoAI）比值与冠状动脉病变严重程度的相关性。方法入选2013年9月至2015年8月期间住院接受冠状动脉造影的2型糖尿病患者245例，根据冠状动脉造影结果分为2型糖尿病合并冠心病组158例和2型糖尿病组87例，所有患者均检测空腹血脂和血糖。运用多因素 Logistic 回归分析 apoB／apoAI 比值与2型糖尿病患者冠心病发病风险的相关性；并通过偏相关分析研究2型糖尿病合并冠心病患者 apoB／apoAI 比值与冠状动脉病变支数以及 Gensini 评分的相关性。结果多因素 Logistic 回归分析发现，apoB／apoAI 比值为2型糖尿病患者发生冠心病的独立危险因素（OR ＝6．196，95％CI：1．417～19．081，P ＜0．001）；应用偏相关分析发现，校正相关因素后，apoB／apoAI 比值与冠状动脉病变支数以及 Gensini 评分均呈正相关（r ＝0．236，P ＜0．001；r ＝0．257，P ＜0．001）。结论ApoB／apoAI 比值对2型糖尿病患者的冠心病发病风险以及冠状动脉病变严重程度的评估均有一定的临床意义。%Objective To explore the relationship between the ratio of apolipoproteinB(apoB)/apolipoproteinAI(apoAI)and the sever-ity of coronary artery lesion in type 2 diabetic patients with coronary heart disease(CHD).Methods In total,245 type 2 diabetic pa-tients hospitalized from September 2013 to August 2015 were enrolled and classified into a type 2 diabetes mellitus with coronary heart disease group (158 cases)and a type 2 diabetes mellitus group (87 cases)according to the results of coronary angiography.All the patients had fasting lipid profile and fasting blood glucose test.Multivariate Logistic regression was applied to estimate the relationship between the ratio of apoB/apoAI and the risk of CHD in type 2 diabetic patient.Partial correlation analysis was used to analyze the cor-relation of the ratio of apoB/apoAI with the narrowing vessel number of coronary artery and Gensini scores.Results Multivariate Lo-gistic regression analysis showed that the ratio of apoB/apoAI was an independent risk factor for occurrence of CHD in type 2 diabetic patients(OR =6.196,95%CI:1.417 ~19.081,P <0.001).Partial correlation analysis showed that the ratio of apoB/apoAI was posi-tively related with the narrowing vessel number of coronary artery(r =0.236,P <0.001)and Gensini scores(r =0.257,P <0.001) after adjusting relevant factors.Conclusion The ratio of apoB/apoAI may have some clinical significance in evaluating the risk of CHD and the severity of coronary artery stenosis in type 2 diabetic patients.
