Erotic transference/countertransference and facilitating discussion of sexual feelings in supervision

The topic of sexual misconduct by therapists has become an area of growing concern within the field of psychology as well as by the public. There is evidence (Pope and Tabachnick, 1993) that graduate training programs do not adequately address sexual arousal in therapy. The purpose of this dissertation is to create a document that will aid in preventing therapists from acting on sexual feelings in the therapeutic relationship. The phenomena of transference and countertransference are reviewed. Specifically, this dissertation examines the possible reasons why a patient or therapist might sexualize the therapeutic relationship. In order to dispel the taboo around discussing sexual feelings, aspects of supervision that facilitate the discussion of erotic material that develops in therapy are also addressed. A clinical case which demonstrates the development of erotic transference and countertransference is presented. The supervision of this case is also reviewed in order to demonstrate the characteristics of supervision that may facilitate or hinder the discussion of sexual feelings. Gaining understanding for why a patient or a therapist sexualizes the therapeutic relationship will create insight into the dynamics of the relationship and a greater understanding of both parties involved. Having this understanding may help the therapist avoid acting out on erotic feelings. Being able to use supervision as a safe place to explore these issues is critical to the training therapist.
