The objective of this project is to demonstrate how a formulation change in an epoxy adhesive produced an unforeseen failure, which in turn delayed company schedule, resulting in thousands of dollars in downtime. Using past performance as an evaluation in the purchasing process can not always predict the quality and likely degree of customer satisfaction.; Far too many times it is assumed that the procuring companies can control their suppliers because of the almighty dollar. In fact, unless there is a supplier measurement system in place that will continuously measure the supplier's performance before procurement and after items have been received (reject rate), companies are only setting themselves up for costly failures.
