In the last fifty years ejection seats have steadily improved fro mthe original seats whose sole purpose was to propel the crew member away fro mthe aircraft to the current fully automatic ejection seats in service today. Today, ejection seats are in operation that can place a crew member under a fully inflated parachute in 2.3 seconds from escape system initiation. In spite of all the advances in this life saving technology, crew members are still losing their lives because they do not initiate ejection or they initiate ejection too late for the system to fully operate. This paper examines some of the factors that have resulted in a crew member failing to initiate ejection in time to save his or her life.
