Electronic health record system is an informal, non-statutory method of surveying, receiving and checking various health parameters for health monitoring. Routine medical checkup is the only way for human to lead a healthy life. Storing and retrieving of medical history of a patient is very essential for proper disease diagnosis. Improper identification of patient may lead to incorrect dosages of medicines, inaccurate lab work, misdiagnosis, results reported for the wrong person etc. An effective centralized health record system was not developed for this. Here, a portable, low cost, light weighted, low power electronic health record system device is proposed to record and monitor health parameters. This system facilitates continuous real time analysis of various patient parameters such as ECG, heart rate, temperature and foot pressure by using various sensors respectively. The collected data are sent to the cloud using Internet of Things (IoT). The IoT helps to connect various sensors with a common platform. GPS helps to track Patient location during an emergency. Medical history of each patient including medical reports is stored in cloud for easy access. RFID tags are also incorporated with this system for patient identification. This device is successfully tested with real time data.
