Molten metal explosions have caused more financial and human loss than any other hazard in the aluminium industry. A company that experienced a severe explosion would inform our industry of the root cause(s) to help prevent recurrence was standard practice. Over the past decade, the willingness to share root cause analysis findings seems to have disappeared. The absence has resulted in our industry becoming less safe and some explosions occurring that might have previously been prevented. This paper will review the root cause analysis of a Force 3 molten metal explosion that occurred in 2016. This information may be used to prevent another explosion from occurring. Without the willingness to openly share root cause analysis findings, aluminium companies are needlessly suffering huge financial losses and aluminium workers are being injured and killed by recurring molten metal explosions.
