Promotion of hazard identification to improve local safety culture

In 2011, all rigs operating for Changbei Development Drilling Campaign achieved a 5-years incident free record. But this achievement was eclipsed by a medical case which occurred just two months later. One of the findings from the incident investigation pointed towards the lack of hazard identification prior to and during the actual job. It was also identified that although 5 years in to the project, the local rig contractor’s crew was focused primarily on the job with lesser priority to safety, as is the prevalent local culture. In order to revive the site safety culture to its former glory, a new program for hazard identification was rolled out in the beginning of 2012 by modifying the already existent hazard hunts. In this program, a schedule was prepared for carrying out hazard hunts on alternate days with 15 hazard hunts planned every month, each focusing on a unique theme. All crew memberswould take turns to participate in these hazard hunts. A checklist was prepared which would guide focus towards certain areas where most hazards could be uncovered. Hazards identified were shared amongst all the crew and pictorial records kept to show a safe and unsafe condition comparison. Almost 9 months after rolling out this program, it was noticed that the number of hazards picked up during these hunts had reduced, and the number of intervention STOP cards had increased. This trend was seen as a result of the local crew’s improved ability to identify hazards during their daily jobs, thus reducing the findings during the prescribed hazard hunts. This was recognized as a step change in how the crew identified hazards and understood the risks. This concept is treated as the stepping stone towards further improvement of the local safety based competency to provide safer operations in all regions.
