Most people suffer significant enough back pain at some point in their lives that their normal daily activities will be disrupted1. Most of these cases resolve with conservative management but approximately 5% or more of these will suffer chronic pain and functional impairment2. As many as 60% of patients may suffer recurrent or persistent low back pain (LBP)3. An estimated 30-50% of all causes of LBP are "discogenic" from Internal Disc Disruption (IDD). In this chapter, we discuss the anatomy, pathogenesis, diagnosis, and treatment of IDD.
