The functional integrity of the anterior floor of the mouth is essential in maintaining tongue mobility and consequent normality of articulation, deglutition, and control and disposal of saliva. Unilateral subcutaneous pedicled nasolabial island flaps permit the reconstruction of defects of the anterior floor of the mouth in a one-stage procedure. Eighteen patients underwent reconstruction of anterior floor of the mouth defects with nasolabial island flaps. The functional and aesthetic outcome is evaluated both from the surgeon's and from the patient's point of view. The unilateral inferiorly and subcutaneously based nasolabial island flap provides reliable coverage of small and intermediate-sized defects of the anterior floor of the mouth, improving tongue mobility, articulation, and deglutition.
