Failure of Inner Shell of Double Walled Atmospheric Ammonia Tank

A 5,000 MT atmospheric ammonia storage tank, which was originally commissioned in March 1998, needed to be decommissioned and repaired due to the failure of the inner shell of this double walled tank. The causes of the failure, as well as the corrective and preventive actions taken, are discussed.
