In 2017, INPO recognized shortfalls in maintenance personnel applying fundamentals and technical skills were resulting in a steady increasing trend of consequential events. These events were causing reactor scrams, reactor power reductions and adversely affecting outages. In order to prevent a significant event, INPO along with industry personnel developed and issued INPO Event Report (IER) L2 17-09 Weaknesses in Maintenance Technical Fundamental Skills Adversely Affecting Plant Operations. A graded approach was used to determine how and when stations reported their actions to INPO. Additionally, INPO designated the adverse trend in maintenance as a key issue requiring a substantial action plan. The purpose of this paper and the resultant presentation is to outline actions taken and planned to significantly improve industry maintenance performance. The INPO key issue action plan focuses on setting standards, measuring performance, facilitating performance improvement and self-regulation to influence and shape industry maintenance performance. Challenges to achieving maintenance personnel applying sound fundamentals to improve equipment reliability include weak preparation and execution, inadequate oversight and insufficient training.
