The root cause analysis (RCA) for a deflagration incident in a large dust collector will besummarized. This includes evidence collection, development of the why tree, additional datacollection and follow up. Findings from this investigation illustrate the hazards of combustibledust and key preventative measures that can be utilized to prevent this type of incident. It alsoillustrates why combustible dust hazards should be considered in process changes made throughmanagement of change (MOC).
