Objective Clinical observation showed lymphopenia in patients with severe acute respiratory syndrome (SARS) .The changes in the immune system during different phases of SARS were studied, Methods T - lymphocyte subsets were measured by flow cytometry during different phases of SARS in 124 patients and compared with those in 51 patients with Acquired Immunodeficiency Syndrome (AIDS) and 57 normal subjects. In addition, the immunoglobulin fraction of each subject was assayed. Results The CD3~+ T - lymphocytes (589.03+-435.12 x10~6/L), CD4~+T- lymphocytes (315. 60 +- 267. 23 x 10~6/L) and CD8~+ T - lymphocytes (214. 64 +-165.36x10~6/L) in the SARS group were significantly lower than the normal group (1162.50+-323.86x 10~6/L,641.34+- 185.86X 10~6/L and 405.46+-146.28 x 10~6/L), respectively. However, the CD4 + T - lymphocyte count was significantly greater than that observed in the AIDS group (153.20 +- 105.23 x 10~6/L) , while the CD3~+T - lymphocyte and CD8~+T- lymphocyte count were significantly lower than that in ADDS group (1122.10 +- 572.05 x 10~6/L and 968.90 +- 546.25 x 10~6/L), respectively. The CD4~+ T - lymphocyte count was < 200 x 10~6/L in the 54 (43.55% ) members of the SARS group and 39 (76.47 % ) members of the AIDS group, respectively. The CD4~+ T- lymphocyte count of all subjects in normal group was at least 200 x 10~6/L. The lowest T- lymphocyte count was obtained 10 and 12 days after onset of SARS symptoms. Both serum IgG (15.14+- 8.51 g/L) and IgE(0.57 +-0.94 g/L) in the SARS group were higher than the normal group(12.41+-2.2 g/L and 0.30+-0.57 g/L). Conclusion The T - lymphocyte subsets decrease during SARS and vary during the course of the disease.
