A security assessment tool that enable the management of a small nuclear or radiological facility to understand properly their PPS functions and weaknesses by using site data will enrich significantly to the recent available tools. This tool will be provided by extended fault tree analysis method that extends its basic event and minimal cutset with malicious action. This proposed method starts with analysing the PPS to built its specific functional block diagram (FBD). The fault tree diagram is drawn from this FBD after defining the important top events (PPS failure). Finally, an evaluation to the fault tree diagram is made. This proposed method has been applied to the PPS of one radiotherapy facility in Indonesian hospital. The defined top event is a succesful unauthorized acces to the Co60 source. The result shows their systematic and logical operational functions and weaknesses to the malicious acts.
