Risk-based judgments in process hazard analyses

The primary product of a hazard analysis is a list of reocmmendations that management can implement to reduce risk.Unfortunately,the process by which analysis teams conclude that the risk of an accident is unacceptable is often highly subjective and inconsistent,as is the process by which management decides whether to implemnet the recommendations.This ppaer describes a simple scoring methodology by which risk-based judgments can be made more consistently,objectively,and defensibly.The methodology can be easily adapted to aid specific risk-based decision,such as determining the required safety integrity level of an instrument system or the need fro additional independent protection layers (decisions faced by all hazard analysis teams).
