Finding an appropriate level of safeguards

There is much discussion of hte appropriate level of safeguards for given situations during process design reviews,process hazards analysis,and in the wake of incidents.Some companies have adopted rather dogmatic rules to simplify decision makig.For example,you often hear the statement "We don't consider double Jeopardy events." This statement assumes all hazards and safeguards are equivalent,lumping together nuisances with catastrophes and low reliability safeguards with high reliability protections.It also flies in the face of experience,in that major losses are primarily a result of multiple failures.
