    • 摘要: Objective To evaluate the accuracy of two electronic root apex locators under in vivo condition.Methods A total of 40 single-rooted maxillary anterior teeth,with normal pulp vitality and withou tany dental diseases,were selected due to prothedontic needs.The pulp chamber was opened by conventional way under local anesthesia.The working length was measured by third generation of electronic root apex locator ROOT ZX,and forth generation VDW Raypex5.The file was then placed into the root canal for CT scan to obtain the position of root apical foramen.The working length was measured and used as gold standard.Results The accuracy of ROOT ZX and VDW Raypex5 was 87.5% and 92.5%,respectively.With introduction of diagnostic evaluation method of classical epidemiology,the Receiver Operating Characteristic (ROC) curve was drawn by spssl3.0 according to measured data,the area under the curve of ROOT ZX and VDW Raypex5 was 0.489 and 0.505,respectively.Conclusion The ROC curve was an effective method for comprehensive,accurate diagnostic evaluation.Both ROOT ZX and VDW electronic root apex locators are accurate,safe and reliable instruments for root canal length measurement.Higher accuracy values were obtained with the VDW Raypex5,but no statistically significant difference was observed (P>0.05).%目的 评价2种根尖定位仪的准确性.方法 选取因修复需要根管治疗的上前牙,共40个根管.要求未罹患任何牙体牙髓疾病,具有正常牙髓活力.局麻下常规开髓,分别选用第三代电子根管测量仪ROOT ZX及第四代产品VDW Raypex5测量工作长度.随后插针进行牙CT拍摄,从牙CT影像中得到根尖孔位置,测得工作长度,并以此作为金标准.引入经典流行病学中对诊断试验评价的方法,采用SPSS13.0软件,根据测得数据绘制受试者工作特性曲线(receiver operator characteristic curve,简称ROC曲线),按曲线下面积,比较几种诊断试验的诊断效率.结果 ROOT ZX和VDW Raypex5的准确率分别为87.5%和92.5%.ROC曲线下面积ROOT ZX为0.489,VDW Raypex5为0.505.结论 ROOT ZX和VDW两代产品都对根管工作长度的测量结果准确、安全、可靠.ROC曲线是一种全面、准确评价诊断试验有效的方法.

    • 摘要: 目的 初步评价根尖定位仪在牙根纵折早期诊断中的应用价值.方法 收集未做过牙髓治疗、临床诊断为牙根纵折或疑似纵折的患牙48颗,根尖定位仪测量根管工作长度.如工作长度较X光片上牙根长度短≥1/3者确诊为纵折.拔除牙根后,测量纵裂根管长度,并与定位仪测量结果进行比较.如定位仪测出的工作长度与X光片上牙根长度相差0.5~1 mm,则行常规小锥度根管预备和冷侧压充填、降合,观察1个月无症状后行冠修复.结果 48颗患牙中,41颗确诊为纵折,拔除后均可见裂隙贯穿根管,确诊率为85.4%.直视下测量的数值与根尖定位仪所测数值相比,差值均在1 mm以内.其余7颗疑似纵折牙中,4颗于修复后0.5~1年发现牙根纵折.结论 根尖定位仪对牙根纵折的早期诊断具有一定的价值,但对于仅有裂线而无裂隙的患牙仍有漏诊的可能性.

    • 摘要: 目的:研究根管治疗术根尖定位仪侧根长中的四手操作护理配合。方法:选取2013-04~2015-04我院收治的200例接受根管治疗的患者作为研究对象,随机数字表法分配观察组与对照组,各100例,对照组行常规护理配合,观察组实施四手操作护理配合,对两组患者治疗满意度、不良反应发生情况、手术治疗时间进行统计并分析,比较护理效果。结果:对照组患者不良反应发生率显著高于观察组,手术治疗时间显著长于观察组,具有统计学意义( P <0.05)。观察组患者治疗满意度显著高于对照组,具有统计学意义(P <0.05)。结论:四手操作护理配合可以有效提高根管治疗术的治疗效果,提升患者治疗满意度,缩短患者治疗时间,可以广泛推广应用。

    • 摘要: 目的:体外评价Propex Ⅱ根尖定位仪在不同根管环境下测定根管再治疗工作长度的准确性。方法选取39颗离体前磨牙,采用标准牙胶尖和AH plus 糊剂恰填根管。建立体外模型,Propex Ⅱ根尖定位仪在不同根管冲洗液中测量根管再治疗后工作长度,与实际工作长度进行比较。结果不同根管内容物下,Propex Ⅱ测量的根管再治疗后工作长度与实际工作长度差异均无统计学意义(P>0.05)。结论在本研究条件下,Propex Ⅱ根尖定位仪在不同根管环境中能够较准确地测定根管再治疗工作长度。

    • 摘要: 目的:研究5.25%次氯酸钠液、生理盐水、17% EDTA、3%过氧化氢液4种不同冲洗液以及血液对ProPex 根尖定位仪测量准确性的影响。方法选择根尖孔发育完成的单根管离体牙45颗,记录理论实际值后在生理盐水琼脂凝胶上构建体外评价模型,根管内分别灌注5.25%次氯酸钠液、生理盐水、17% EDTA、3%过氧化氢液和血液,使用Propex 记录测量值。结果各组根管内容物对Propex 测量值的影响无统计学意义(F =1.959,P =0.090,P >0.05)。结论 ProPex 测量牙齿操作长度的准确性不受根管内容物的影响。%Objective To study the effect of four different solutions:5. 25% NaOCl,0. 9% NaCl,17% EDTA,3% H 2 O2 and the blood on the accuracy of ProPex apex locator. Methods Forty-five extracted teeth with single straight canals and completed foramina were selected to record the ideal actual measurements and then were taken on a normal saline agar model to mimic the situation in vivo. Forty-five single canals teeth filled with 5. 25% NaOCl,0. 9% NaCl,17% EDTA,3% H 2 O2 and blood were used to record the experi-mental measurements with the ProPex apex locator. Results The accuracy of ProPex apex locator was not affected by different canal contents and the measurements of each group were not statistically different(F = 1. 959,P = 0. 090,P > 0. 05). Conclusions Canal length measuring accuracy of ProPex is not affected by the contents of the root canal.

    • 摘要: Objective To compare the accuracy of PropexⅡand Raypex 5 electronic apex locators in mea-suring root canal working length. Methods 80 teeth, with a total of 135 root canals, were selected to take the root ca-nal therapy. The root canal working length was measured by PropexⅡapex locator and Raypex 5 apex locator respec-tively, recorded the numbers, and then inserted the reamer into root canals according to the measured length, took x-ray dental film, observed the relationship between the root tip and tip of file using the cliniview 5.0 software. The ac-curacy rates of two apex locators were calculated basing on the results obtained. Results The accuracy rate of PropexⅡapex locator in measuring root canal working length was 91.1%, while Raypex 5 apex locator was 92.6%, and there was no statistically significant difference between the two groups (P>0.05). Conclusion Both PropexⅡapex locator and Raypex 5 apex locator could measure the root canal working length accurately, therefore are worth clinical popu-larizing and application.%目的:比较PropexⅡ和Raypex 5两种根尖定位仪测定根管工作长度的准确性。方法选取拟行根管治疗的患牙80颗,共135个根管,分别用PropexⅡ和Raypex 5两种根尖定位仪测定根管工作长度,记录数值,然后将扩大针按测得的工作长度插入根管内,拍摄牙片,运用Cliniview 5.0软件系统观察锉尖与根尖的关系,依据所得出的结果计算两种根尖定位仪的准确率。结果 PropexⅡ测定根管工作长度的准确率为91.1%, Raypex 5测定根管工作长度的准确率为92.6%,两者间的差异无统计学意义(P>0.05)。结论 PropexⅡ和Raypex 5两种根尖定位仪均能较准确的确定根管的工作长度,值得临床推广应用。

    • 摘要: 目的 比较研究Woodpex Ⅲ、Propex、DENTAPORT ZX根尖定位仪在临床上对不同情况的根管长度判断的准确性,分析其临床适用范围及影响因素.方法 97颗牙175个根管依据病情不同,在初步完成根管上端预备后,使用Woodpex Ⅲ、Propex、DENTAPORT ZX根尖定位仪测定根管长度,然后,插针照X片检查.结果 Woodpex Ⅲ、Propex、DENTAPORT ZX根尖定位仪测量准确率均为97.71%.三组准确率的差异无统计学意义(P>0.05).结论 Woodpex Ⅲ、Propex、DENTAPORT ZX根尖定位仪测量根管长度具有较高的准确性.

    • 摘要: 目的 评价Dentaport ZX测量根管工作长度的准确性,为临床应用提供参考依据.方法 选取临床中需拔牙的30颗患牙为研究对象,首先对患牙开髓,Dentaport ZX测量根管工作长度,然后拔除患牙在镜下直视测量根管实际长度并以此长度为标准,计算Dentaport ZX电测法与实际根管长度的差值.结果 Dentaport ZX电测法测量值与拔牙实测值差值为(0.214±0.542) mm.以1 mm内为可接受范围,电测法准确率为98.7%,差异无统计学意义.结论 Dentaport ZX能准确测量根管长度,可以指导临床操作.
