    • 摘要: Introduction: In emergence a COVID patient with multisystem inflammatory syndrome in children (MIS-C), associated with SARS-CoV-2, has shown increasing number among pediatric. Even the same presentation of Kawasaki disease we have to keep in mind MIS-C, in order to reach a consensus on this new disease in the future. Three variations of the disease patterns were reported: a group of children with increase in inflammatory activity and persistent fever, without criteria for Kawasaki disease, a second group with Kawasaki disease criteria, and a third group with shock, coronary aneurysms, severe cardiac dysfunction, and gastrointestinal symptoms. Classic Kawasaki disease is self-limited vasculitis which affects medium-sized vessels, almost occurred in children under five years old. Here, we bring a Saudi case, present to emergency department in Prince Sultan Military Medical City. Case report: 10 years old female, medically free, presented with abdominal pain, fever and skin rashes. Patient had history of COVID-19 infection 1 month before presentation. Initial investigations showed acute kidney injury with elevated inflammatory markers. Conclusion: Further evidence of the increase in the incidence of pediatric MIS-C, temporarily is associated with SARS-CoV-2. Physician should give more attentions to this new diagnosis with more fatal outcomes than Kawasaki cases.

    • 摘要: Amebiasis is a parasitic infection caused by the protozoa Entamoeba histolytica. It is capable of causing a spectrum of illnesses from asymptomatic infection, to dysentery and invasive extra-intestinal conditions, the most common of which is liver abscess. Here is presented the case of a 14-month old boy presented with Fever of Unknown Origin, which after work-up was diagnosed as amebic liver abscess. Although amebiasis is more prevalent in developing countries the increasing phenomena of migration and tourism has transformed it into a global health issue. It is recommended to maintain a high index of suspicion while valuating a child with Fever of Unknown Origin.

    • 摘要: Objective: To investigate the changes in the hypothalamic metabolites of lipopolysaccharide(LPS) febrile young rabbits after the treatment with pediatric tuina.Methods: A total of 30 young rabbits were randomly assigned into three groups: the normal group, the model group, and the tuina group. Both the model group and the tuina group were injected intravenously with LPS. “Six antipyretic manipulations”(pushing Tianmen, pushing Kangong, kneading Taiyang,kneading Erhougaogu, clearing Tianheshui, and pushing Jizhu) were administered 1 h after the LPS injection in the tuina group. The rectal temperatures of the young rabbits were monitored during the experiment to explore the antipyretic effect. Three hours after the injection, the content of interleukin-1β(IL-1β), tumor necrosis factor(TNF)-a, and prostaglandin E;(PGE;) in the serum was detected. In addition, liquid chromatography-mass spectrometry(LC-MS) was used for the hypothalamus metabolomics.Results: Compared with the model group, the rectal temperature of the tuina group was decreased at 2 h and 3 h after the LPS injection(P =.04, P =.03, respectively), and the content of IL-1β, TNF-a, and PGE2was decreased(P =.03, P =.003, and P =.008, respectively). The metabolomics results showed that there were 23 potential biomarkers after the tuina intervention, enriching 27 pathways. Lipid metabolites,especially glycerophospholipids, were a majority of the altered metabolites. The primary metabolic pathways affected by tuina included the arachidonic acid metabolism, the GABAergic synapse, D-glutamine and D-glutamate metabolism, and the glutamatergic synapse.Conclusion: Pediatric tuina reduced the temperature of the febrile rabbits and downregulated the expression of IL-1β, TNF-a, and PGE2, and the antipyretic mechanism may be related to changes in metabolites and metabolic pathways in the hypothalamus.

    • 摘要: Background: Malaria has historically been a major public health concern in Yemen, noted to be the country with the highest prevalence in the Eastern Mediterranean Region. As more and more children attend school, governments are increasingly recognizing the importance of child health for educational achievement. Aim: The aim is to assess the baseline knowledge, attitude and practice of pupil towards malaria before the health education activities. Methods: We conducted a cross-sectional study, in four randomly selected districts (rural and urban) of Taiz governorate. The study population was 1598 pupils in primary schools. Data was collected using questioner. Results: The pupils who recognized mosquito bite as a route of malaria transmission was 769 (48.1%), while the pupils who recognized fever as the main symptom were 786 (49.2%), 281 (17.58%) of pupils recognized convulsion as a complication of malaria. Regarding protective method 881 (55.31%) were mentioned the cover of the tanks as a protective method, about the BN 293 (18.3%) of pupils known about the BN and 280 (17.5%) mentioned children & pregnancy were the vulnerable groups to malaria. Conclusions and Recommendation: the percentage of knowledge of malaria knowledge and methods of prevention was low (24.5%). Also, the positive attitude and practice toward malaria was 45%. The rate of having bed-nets was very low (10.1%). There were misconceptions of malaria cause and transmission (48.1%). We recommend conducting health education activities that will focus on increasing the knowledge, attitude and practice levels of malaria among school pupils.

    • 摘要: Abdominal distention after cardiopulmonary resuscitation(CPR)is a common phenomenon that presents in the emergency department.It is caused by a long period of bag-valve mask ventilation or esophageal intubation.However,a rare but life-threating diagnosis should be considered when distention progresses rapidly.Here,we reported a patient who developed a fever and abdominal pain that couldn’t be relieved by gastric decompression.

    • 摘要: Rat-bite is uncommon worldwide, excluding developing countries. The history of a bite is usually missed if not meticulously asked for in history, proper exposure and examination. Today we report a case of culture-negative rat-bite fever with poly septic arthritis resembling a rheumatoid arthritis-like picture in a 64-year-old gentleman. The presentation at first imitated an arthritis-like picture which was polyarthritis affecting the right ankle, right knee, right wrist, right proximal interphalangeal, left metacarpophalangeal and proximal interphalangeal joints.

    • 摘要: BACKGROUND All drugs have the potential to cause drug-induced lung injury both during and after drug administration.Acetaminophen has been reported to cause druginduced lung injury,although this is extremely rare.Herein,we present an extremely rare case of acetaminophen-induced pneumonia.CASE SUMMARY A healthy 35-year-old Japanese woman visited a neighborhood clinic with complaints of fever and malaise following a tick bite.Her treatment included1,500 mg acetaminophen(Caronal^(?)) and subsequently minocycline(200 mg) and acetaminophen(2,000 mg;Caronal^(?)) daily when her condition did not improve;the patient was eventually hospitalized.The patient’s chest computed tomography(CT) revealed consolidation and ground-glass opacities in the right middle and lower lobes.Minocycline was shifted to sulbactam/ampicillin.However,her fever did not improve during follow-up,and her chest CT revealed extensive ground-glass opacities in the right middle and lower lobes and thick infiltrative shadows in the bilateral basal areas.Drug-induced lung injury was suspected;hence,acetaminophen was discontinued.The fever resolved immediately,and inflammatory response and respiratory imaging findings improved.A drug-induced lymphocyte stimulation test was performed against acetaminophen(Caronal^(?)),and significant proliferation of lymphocytes was noted only for acetaminophen(stimulation index,2.1).CONCLUSION Even common drugs such as over-the-counter drugs can cause drug-induced lung damage.

    • 摘要: BACKGROUND Hand,foot,and mouth disease(HFMD)has become one of the most common infectious diseases in China.Before 2016,the primary causal serotypes were enterovirus A71(EV-A71)and coxsackievirus A16(CV-A16).Following the introduction of EV-A71 vaccines in China since 2016,the situation could change.CV-A6 has recently replaced EV-A71 and CV-A16 in some areas of China.However,the epidemiological characteristics of central China remain unknown.AIM To investigate the clinical symptoms and pathogen spectrum of HFMD in Shiyan City,central China,in recent years.METHODS The epidemiological,clinical,and laboratory data from HFMD cases reported to the Shiyan Center for Disease Control and Prevention between January 2016 and December 2020 were analyzed.196 throat swab specimens were collected from hospitalized HFMD patients between January 2018 and December 2020.To detect and genotype enteroviruses,real-time reverse transcription-polymerase chain reaction and sequencing of the 5'-untranslated region were used.In Shiyan,168 laboratory-confirmed HFMD cases were studied using a logistic regression model to determine the effect of predominant enterovirus serotypes.Based on the logistic regression model,the least absolute shrinkage and selection operator model was used to analyze the correlation between CV-A6 infection and various clinical characteristics in HFMD patients in Shiyan.RESULTS From 2016 to 2020,35840 HFMD cases were reported in Shiyan.The number of cases decreased by 48.4%from 2016 to 2017.Approximately 1.58-fold increases were found in 2018 and 2019 when compared to the previous year,respectively.In 2020,a decrease of about 85.5%was reported when compared to 2019.The most common serotypes shifted from EV-A71 and CV-A16(about 60%-80%in 2016 and 2018)to others(more than 80.0%in 2017,2019,and 2020).EV-A71 lost its dominance in 2017 in Shiyan.Among 196 confirmed HFMD cases,85.7%tested positive for enterovirus,with CV-A6 being the most common serotype(121/168,72.0%).The positive rates for CV-A16 and CVA10 were 4.8%and 3.0%,respectively.There was no EV-A71 discovered.Infection with CV-A6 was linked to fever,myocardial damage,increased creatine kinase MB isoenzyme,and lactate dehydrogenase levels.CONCLUSION CV-A6 was the most common enterovirus serotype in Shiyan City,replacing EV-A71 and CV-A16 as the HFMD pathogen.Developing vaccines against CV-A6 or multiple pathogens,as well as rising CV-A6 surveillance,will help prevent HFMD in central China.
