二维斑点追踪技术在评价冠心病患者CABG前后左心功能变化中的应用

         

摘要

目的 探讨二维斑点追踪(STI)技术评价冠心病患者(CAD)冠状动脉搭桥(CABG)术后左心局部及整体功能变化的价值.方法 对32例CAD患者(CAD组)分别于CABG术前、术后3个月行超声心动图检查,应用STI获得左室六个壁基底段、中间段、心尖段的收缩期纵向峰值应变值(SLs)、整体纵向峰值应变(GSL)、舒张早期纵向峰值应变率(SrLe),同时设30名健康志愿者为对照组.结果 与对照组比较,CAD组术前心肌缺血节段SLs、SrLe及GSL减低(P均<0.05);CAD组术后3个月相应节段的SLs、SrLe及GSL均较术前显著提高(P均<0.05).结论 STI技术可定量、客观的评价左心功能变化,此为CABG疗效评定和CAD预后判定提供了一种无创手段.%Objective To investigate the value of regional and global left ventricular function after coronary artery bypass grafting ( CABG) by speckle tracking imaging ( STI) . Methods Transthoracic echocardiography ( TTE ) was performed in 32 patients with coronary artery disease ( CAD) preoperative and 3 months after CABG. Peak systolic longitudinal strain (SLs) , early diastolic longitudinal strain rate (SrLe) , global longitudinal peak systolic strain (GLS) at middle, basal and apical ventricule were measured using STI. The above data of another 30 healthy volunteers were measured as control. Results Compared with the control group, SLs, GLS and SrLe in part of myocardial ischemic segments were lower in CAD group before CABG ( P < 0. 05) ; SLs, SrLe and GSL in the corresponding segments of patients 3 months postoperative showed significantly improving than those indexes preoperative (P<0.05). Conclusion STI can assess the change of left ventricular function after CABG quantitatively and objectively, and provide an effective and noninvasive method for clinical evaluation of left ventricular function.
