对ICU临终患者终止治疗家庭会议讨论和沟通的满意度调查与分析

摘要

Objective To confirm patients' and their families' characteristics, and to list the related record of increasing satisfaction degree and family conference discussion on terminating treatment of terminal patients in ICU. Methods Cohort study was done on terminal patients in ICU of 5 hospitals in Hebi Area. The survey results include total score of discussion and communication satisfaction of terminating treatment, and individual score of support from it. Results Satisfaction of discussion and communication of terminating treatment associates with the abandon of life support, patient charts, which is suggested by doctors to abandon life support, discussion about patients' willingness, and the spiritual need of the family. In the process of decision, support experience is relevant with the abandon of life support, physician care, record chart, which is suggested by doctors to give up life, the expression of family volunteering to abandon, and the spiritual needs from their family. Conclusion The increasing satisfaction of family conference discussion and communication of terminating treatment of terminal patients is relevant with the abandon of life support, and record of palliative care, which includes the suggestion of terminating life support from patients, the expression of patients' willingness and the discussion of the spiritual needs of their family.%目的:确定患者和家属特征并以图表的方式列出治疗过程中与ICU患者家属对终止治疗的家庭会议讨论和沟通满意度提高相关的记录.方法对鹤壁地区5家医院的ICU死亡患者进行了一项队列研究.家属调查的内容包括家属对终止治疗的家庭会议讨论和沟通满意程度的总评分以及反映终止治疗的家庭会议讨论和沟通过程中支持感受的单项评分.结果家属对终止治疗的家庭会议讨论和沟通的满意度与生命支持的放弃、医师建议放弃生命支持的图表记录、患者意愿的讨论和家属精神需要的讨论有关.决策过程中的支持感受与生命支持的放弃、给予精神关怀、医师建议放弃生命支持的图表记录、家属自愿放弃生命支持的表述以及家属精神需要的讨论有关.结论家属对终止治疗的家庭会议讨论和沟通满意度的提高与放弃生命支持以及姑息治疗记录中医师对放弃生命支持的建议、患者意愿的表述、家属精神需要的讨论有关.
