Product development and design change is inevitable but introduces new risks. Experience shows that design weaknesses, including some that affect safety, are not always identified before service operation starts. By the use of practical examples this talk will look at why it is so difficult to catch everything, and draw some conclusions about how to maximise the effectiveness of the safety assurance process.
