The cleft nose has been and will be a challenge to the plastic surgeon. The severity depends of the type of cleft lip, and there isn't any technique that can be accepted as universal model for correction of the nose. During a period of 3 years (2004-2006) 23 cleft unilateral patients had carried through the secondary rhinoplasty, pursuing varied of 1 the 3 years, with average of 2,3 years. The results of the surgery had been evaluated subjectively by reply of the patients, critical analysis of three surgeons through photos before and after the surgical procedures and medical data. The opened rhinoplasty was an excellent surgical access and supplies a good exposition of the deformed structures. The use of the structural cartilage grafts was of great value with good aesthetic results for the patient and satisfaction of the surgical team.
