Sepsis treatment and acute blood purification: sepXiris~?

Sepsis and septic acute kidney injury are important and burdensome syndromes, with poor prognosis and high cost. Blood purification offers an opportunity for organ support rather than simply organ replacement, through correction of the dysregulated immune response that is so prominent in these conditions. sepXiris? is a cytokine adsorbing membrane that can be used for continuous and prolonged intermittent renal replacement therapy. In published and peer-reviewed clinical reports, sepXiris? is associated with rapid reduction in levels of circulating cytokines, and clinical outcomes that are better than predicted from illness severity score. In a substantial body of unpublished conference reports on file, sepXiris? is clearly a promising tool for sepsis and septic acute kidney injury, with reports of benefit across a wide range of settings. Future research is needed to define the optimal timing and application of this tool, and initially should probably take the form of a large and detailed multicenter disease registry.
