A 68-year-old man with a history of dementia and hypertension presented with pneumonia and an incidental finding of dilated coronary arteries. Coronary CT angiography showed an anomalous origin of the right coronary artery (RCA) from the pulmonary trunk with marked dilatation and tortuosity of the coronary arteries and with an extensive collateral network between the left coronary artery (LCA) and RCA systems.
