Progressive myelopathy with hyperintensity on T2-weighted spinal MRI.

1. Clinical background:A 64-year-old woman was admitted to our hospital with progressive gait disturbance and urinary, dysfunction. At 62 years old, she was diagnosed with a flaccid neurogenic bladder and started practicing intermittent self-catheterisation. She had developed a slowly progressive gait disturbance and had become wheelchair-bound before admission. Neurological examination revealed spasticity of the lower limbs, a flaccid neurogenic bladder, and mild deep sensory disturbance. A cerebrospinal fluid (CSF) study revealed normal cell count (5 cells/mL) and immunoglobulin (Ig)G index (0.64). Cervical T2-weighted MRI with gadolinium enhancement revealed an increased high-signal-intensity region extending from C2 to C5 (Fig. 1). The brain and thoracic MRIs were normal.
