Vertebral arterial dissection (VAD) is an important cause of stroke in young patients, and may follow a traumatic precipitating event such as chiropractic manipulation.1,2 We report a patient who developed VAD after dancing the samba. His sensations at onset were characteristic and may aid in the early diagnosis of VAD.One evening, a non-diabetic, normotensive 36-year-old man with a history of smoking, danced a particularly vig-orous type of samba (Matsuken-samba), recently popular in Japan. One hour later, while resting, he suddenly heard a sound in the left side of his neck like 'puchi' (the sound that something is cut) followed immediately by a sound like 'bubbles being lost from carbonated water'. Immediately, he experienced dysethesia on the left side of his body and pain and stiffness in the left occipital region, neck and left shoulder. He also became nauseous and vomited. He was taken to a nearby hospital by ambulance.
