A previously healthy 22-year-old female patient was transferred to a quaternary referral centre for consideration of neuro-surgical care following a witnessed generalized tonic-clonic seizure with associated urinary incontinence. The patient received a loading dose of intravenous phenytoin and a CT scan of the brain was performed (Fig. 1).
