经脐单孔腹腔镜加穿刺针在精索静脉曲张治疗中的应用

摘要

目的 探讨经脐单孔腹腔镜加穿刺针治疗精索静脉曲张的临床疗效.方法 选择精索静脉曲张患者35例,采用经脐单孔单通道腹腔镜,手术仅在脐部切开1个1.0 cm穿刺通道,在下腹壁穿刺针(气腹针Veres内芯)的辅助下完成腹膜后精索血管的高位集索结扎.结果 35例均顺利完成手术,术后住院时间2～4d、平均3d.随访2 ～18个月,无静脉曲张复发、阴囊水肿,无睾丸萎缩等远期并发症.结论 单切口单通道腹腔镜下治疗精索静脉曲张经济、简便,临床疗效显著.%Objective To investigate the clinical effects of transumbilical single port laparoscopic varicocelectomy combined with an inserted needle.Methods 35 patients with grade Ⅱ-Ⅲ varicocele were undergone transumbilical single port laparoscopic high ligation of spermatic vein.The pneumascos needle was inserted in the middle of the incision to form pneumoperitoneum.A 10 mm trocar was placed and connected to the pneumoperitoneum machine.Both the artery and the vein were dissected and ligated with intracorporeal knot-tying above the internal inguinal ring.Results All of the operations were performed successfully and no complications had been noted.All patients resumed normal activity within 2-4 days.Mean postoperative hospital stay was 3 days.Neither hydrocele nor testicular atrophy was observed postoperatively.Conclusion Transumbilical single port laparoscopic treatment with high ligation of spermatic vein is effective in treating varicocele.
