Objective To analyze the diagnostic value of T-SPOT. TB test and tuberculosis antibody test on extrapulmonary tuberculosis. Methods From April 2014 to March 2016,a total of 173 suspected extrapulmonary tuberculosis patients were selected in the Hospital of Jiuyuan District,Baotou,finally 125 cases diagnosed as extrapulmonary tuberculosis, 48 cases were not extrapulmonary tuberculosis;all of them received T-SPOT. TB test and tuberculosis antibody test,positive detection rate and diagnostic value were compared between the two methods. Results We take the bacteriological examination results of sputum specimen as the gold standard for extrapulmonary tuberculosis,the positive detection rate of T-SPOT. TB test was 73. 99% ,that was statistically significantly higher than of tuberculosis antibody test of 62. 43% (P ﹤ 0. 05). The sensitivity of T-SPOT. TB test in diagnosing extrapulmonary tuberculosis was 99. 20% ,the specificity was 91. 67% ,the diagnostic accordance rate was 97. 11% ;the sensitivity of tuberculosis antibody test in diagnosing extrapulmonary tuberculosis was 81. 60% ,the specificity was 87. 50% ,the diagnostic accordance rate was 83. 24% ;the sensitivity,specificity and diagnostic accordance rate of T-SPOT. TB test was statistically significantly higher than that of tuberculosis antibody test in diagnosing extrapulmonary tuberculosis,respectively(P ﹤ 0. 05). Conclusion The sensitivity,specificity and diagnostic accordance rate of T-SPOT. TB test are statistically significantly higher than those of tuberculosis antibody test in diagnosing extrapulmonary tuberculosis.%目的：分析结核感染 T 细胞斑点（T-SPOT. TB）试验与结核抗体试验对肺外结核的诊断价值。方法选取2014年4月—2016年3月包头市九原区医院收治的疑诊肺外结核患者173例（最终确诊为肺外结核125例，非肺外结核48例），分别进行T-SPOT. TB试验及结核抗体试验，比较两种方法的阳性检出率及诊断价值。结果以痰液标本细菌学检查结果作为诊断肺外结核的金标准，T-SPOT. TB试验对肺外结核的阳性检出率为73.99%，高于结核抗体试验的62.43%（ P ﹤0.05）。T-SPOT. TB试验诊断肺外结核的灵敏度为99.20%，特异度为91.67%，诊断符合率为97.11%；结核抗体试验诊断肺外结核的灵敏度为81.60%，特异度为87.50%，诊断符合率为83.24%；T-SPOT. TB试验诊断肺外结核的特异度、灵敏度、诊断符合率高于结核抗体试验（P ﹤0.05）。结论 T-SPOT. TB试验诊断肺外结核的灵敏度、特异度和诊断符合率高于结核抗体试验。
